Please indicate in the "Notes" section who your Insurance Provider is if you wish our office to provide Direct Billing for your Massage. We may contact you with special instructions in order to complete this request.
Please note that we are not able to provide direct billing to every insurance company. On our home page is a complete list of companies we currently provide direct billing to. We also ask that you check your Massage Therapy coverage with your Insurance Provider prior to your appointment. Not all plans allow for direct billing, or they may have restrictions to Massage Therapy coverage. We are not able to check coverage details on your behalf.
We also ask that you do not wear heavy perfume/cologne when visiting our clinic. We have many clients & some staff who are sensitive/allergic to these products. For their comfort & well being we ask that you try to avoid these heavy scents. Thank You!
As of January 1, 2020 we will require a credit card number on file in order to book appointments on long weekends, statutory holidays, and December 19-31, 2020. This change is in response to a high volume of no-shows during these busy days. Failure to provide 24 hours notice to cancel your appointment will result in a full charge for the missed appointment. We apologise for any inconvenience this may cause.
