COVID-19 has forever changed the way our clinic operates. On top of our usual strict cleaning protocols; we have converted the clinic to run paperless in an effort to reduce high touch areas that cannot be disinfected. Because of this, we ask that all clients complete a new Medical History, Consent Form & Electronic Transmission Authorization form so that we can update your personal file. These forms will be emailed to you at the time of your booking.
You will also be asked to complete a COVID-19 Screening form at the beginning of every appointment. All TMK staff will also be screened for COVID-19 symptoms prior to their shift every day.
You must cancel your appointment if you are feeling ill or if anyone in your household is ill, even with mild symptoms and regardless of your vaccination status!! No late charges will be applied for cancellations due to illness at this time, though we still ask you to provide 24 hours notice when possible. Failure to show up for your appointment without notice will still be subject to a late cancellation fee.
